Rehabilitation Services

The Rehabilitation Department provides Physical Therapy, Occupational Therapy, and Speech Therapy evaluations and treatment for residents/registrants in our Nursing Home and Adult Day Services. We strive to optimize movement, strength, communication and functional independence while eliminating or minimizing their pain.

Physical Therapy:

Our Rehabilitation Department partners with Adirondack Physical Therapy and Sports Rehabilitation PC in order to provide the highest quality of care possible. No other physical therapy practice in the area can equal the combination of certifications, training, experience and expertise that the staff of Adirondack Physical Therapy & Sports Rehabilitation offers.

The Physical Therapy staff at Evergreen Valley Nursing Home treat pain, disease and injury; assist in recovery from surgery with specially designed exercises and equipment to help restore movement and mobility; promote independent living; and provide education in the prevention of injuries.

Our therapists are dedicated to helping residents achieve their highest level of independence. Collaborating with physicians and utilizing various modalities, techniques and treatment approaches allows us to safely return residents to their homes, or other appropriate living arrangements, as soon as possible. Goals are assessed weekly in order to monitor the treatment plan to ensure clinical needs are met and/or exceeded. Once rehabilitation goals have been achieved, discharge planning occurs including home assessments and in-depth caregiver training to support a resident's continued success in their own environment.

  • Reduction of pain
  • Improvement of body movement
  • Maintenance of Mobility
  • Overall improvement in health

Occupational Therapy:

Occupational Therapy is designed to enable people with limitations or impairments to participate in the activities of every day life. At Evergreen Valley Nursing Home our Occupational Therapist evaluates and treats individuals whose ability to function may have been limited or diminished through normal aging, illness, acute injury, elective surgery or developmental disability. Treatment goals include functional independence, prevention of disability and maintenance of wellness.

Our OT services include:

  • Education and training in daily living skills such as bathing, dressing, and eating
  • Therapeutic adaptations, such as assistive equipment and physical environment design
  • Sensorimotor treatment in strengthening, endurance, range of motion, coordination balance and functional mobility
  • Prevention and health promotion through energy conservation, body mechanics and joint protection/preservation
  • Compensation for sensory loss

Speech Therapy:

Speech-Language Pathologists identify, evaluate and treat a wide range of speech, language and swallowing disorders resulting from a variety of diagnoses such as Parkinsonís Disease, laryngectomy, stroke, head injury, memory impairments, voice dysfunctions, dysphagia and stuttering.

Treatment goals in Speech/Language/Swallowing Therapy can target improved verbal/written expression, auditory/reading comprehension, memory, voice, fluency and the ability to improve swallowing. Our Speech Therapist works with the resident, care team and family members to prevent impairments, to target meaningful goals to improve function and maintain skills taught during therapy.

Examples of Speech/Language/Swallowing Services at Evergreen Valley Nursing Home:

  • Aphasia Therapy (impaired use or comprehension of language) to improve communication skills after a stroke or other neurological incident
  • Voice Therapy to improve vocal quality, breath support and to prevent vocal pathologies
  • Dysarthria/motor speech therapy (articulation) to improve speech production and articulation
  • Evaluation and training of augmentative communication (non-verbal communication aides) such as communications boards, memory books, or speech generating devices.
  • Dysphagia (swallowing) therapy including therapeutic exercises
