How does The Adult Day Services at Evergreen Valley help families?

Evergreen Valley offers ďthis missing linkĒ in assisting and supporting families with the care of their loved ones. We support family values by providing a community-based, quality care program that allow individuals to remain in their own homes and/or with their chosen care givers.

Our program offers an option and choice for individuals requiring health, rehabilitative, therapeutic and palliative care. It is an alternative to avoid early or inappropriate nursing home placement. The Adult Day Services gives families and care givers peace of mind by offering emotional and physical support to promote the individualís maximum level of independence.

What is Adult Day Health Care?

Adult Day Health Care is a community-based long-term care program that provides comprehensive health care to individuals with functional impairments, chronically ill and the frail elderly in a congregate day setting.

What makes The Adult Day Services special?

Our Adult Day Services is the only daytime "medical model" program in our area. We provide intervention that reduce incidences requiring hospitalization and that promote wellness.

Case management within the Adult Day Health Care environment is an integrated process involving:

  • Comprehensive care planning by an interdisciplinary team;
  • Provision of services based on the prescribed orders of the registrantís personal physician;
  • Professional assessment and monitoring to ensure positive health outcomes;
  • Coordination of our registrantís health needs with other community agencies.

Case management is an indispensable function that exemplifies the holistic philosophy of Adult Day Health Care and, as such, is ongoing. Whether or not the registrant is in attendance. It is this unique program feature which serves as the cornerstone of Adult Day Health Careís cost-effective provision of quality care.

Who does Adult Day Services employ?

Our team of dedicated, friendly staff consists of a registered professional nurse, certified dietician, social worker, licensed practical nurse, certified nursing assistants, transporter/driver, secretarial support.

Rehabilitation therapies

  • Kim Orrichio, PT and her staff from Adirondack Physical Therapy and sports rehabilitation, P.C. provide physical therapy during program hours to registrantís who have a physician order for service.

When is the Center open?
Our program is five days a week, Monday through Friday, 7:30 a.m. to 5:00 p.m.

Are there different rooms on different floors?

The Adult Day Services entrance is separate from the nursing home. We have a ground floor plan of 4,900 square feet and this allows easy accessibility to the outside and vegetable garden.

The Great room is where a majority of activities are conducted; nutritious, therapeutic, delicious lunch is served in this room.

The treatment room allows privacy for medical assessments, treatments, dressing changes, administration of medications, evaluation by outside health care professionals working with our registrants (OT, speech therapist, public health nurse)

The Activities/Conference room is another area for registrants to work on individual or small group projects. It is a quiet, private area for registrant care plan meetings.

The Tub room and separate Shower room allows registrants the ability to have personal care provided on-site as part of their program time by our certified nursing assistants.

There is also a Beauty Shop on-site with full services offered by a licensed cosmetologist at a nominal fee.

Do Registrants of the Adult Day Services have any say as their plan of care?
Registrants are encouraged to participate in their plan of care as well as monthly Registrant Council meetings.

How is the Adult Day Services paid for?
This Adult Day Services is licensed by the New York State Department of Health and can be paid for by Medicaid, some private insurances or directly by the registrant.
